No More Room in Hell 2 features a deadly infection mechanic that differs from the original game.

What Causes Infection?

You can become infected when a zombie physically attacks you, especially when your health is around 50% and lower; each hit carries a small chance that rises as your health drops. Zombies may also grab you, triggering a quick‑time event that, if failed or performed while low on stamina, instantly infects you. Shambler zombies release a poisonous cloud when killed, and any zombie that walks through the gas becomes empowered, both of which have a high chance of causing infection on contact or attack. To stay safe, use firearms to eliminate Shamblers and other infected zombies from a distance.

All Stages Of Infection

After infection you have a few minutes before symptoms appear, progressing through three stages. Stage one causes visual distortions such as blurred screen edges, white lines and discoloration. Stage two adds audio echoing, coughing from your Responder, and alerts nearby players. Stage three brings intense hallucinations, louder coughing, visual apparitions, red veins crawling across the screen, and a death cue before you succumb. Dying from infection turns you into an aggressive sprinting zombie that threatens your teammates.

How To Manage Infection

Infection can be managed with Phalanx Pills, which reset the infection timer. These pills are found in medical shelves, offices and supply containers; each container holds three uses and is best saved for moments when you are near full infection. Pills can only be used while infected, so they also serve as a quick check for infection status. Saving pills for later stages is efficient, but using them mid‑combat can be risky due to stamina loss and hallucinations.

How To Fully Cure Infection

Rare Gene Therapy kits remove the infection and grant immunity for the rest of the match, though they require a 10‑second charge after injection. If a kit is unavailable, extracting from a mission instantly cures all infection stages. Early extraction is advisable if you become infected early or lack pills. When out of options, you can use the self‑sacrifice feature: press K on PC or hold the right bumper and left trigger on consoles with a loaded firearm to kill your Responder, preventing the zombie form from harming allies. On PC a accidental tap will instantly result in the death of your character, so use caution. Only Hard and Nightmare difficulties result in permanent character death when a Responder dies, making it a good idea to familiarize yourself with the steps and warning signs before tackling the tougher challenges that No More Room In Hell 2 has to offer.
